Microneedling and Minoxidil: What the Studies Found

Most studies of microneedling for hair loss pair it with minoxidil. Some found the pair beat minoxidil alone and the largest recent trial did not. This page sets out what was tested and what to ask before combining anything.

Scope. Minoxidil is a drug. We describe studies only. We give no doses, schedules, products or brands, and we do not recommend any drug. Hair loss has medical causes, so see a dermatologist first.

Why the two are studied together

A 2018 expert review says microneedling is thought to help first-line medicines get through the skin, and that this may be one way it promotes hair growth. It adds that the current evidence does not allow a claim that microneedling beats standard therapies. The FDA, for its part, says it has not reviewed the safety or effectiveness of microneedling devices used together with other products.

What the combination studies found
StudyWho took partWhat was measuredLimits
Randomized pilot, 2013
Pilot or small study
100 men.Weekly microneedling plus a drug lotion against the lotion alone, 12 weeks. Average gain in a marked 1 cm² patch: 91.4 hairs against 22.2.Small and short. Evaluators were blinded.
Randomized trial, 2026
Randomized trial
245 women.Minoxidil alone, plus monthly microneedling, plus microneedling every 2 weeks, 24 weeks. All groups gained hair. No significant difference between them.Single center, open label.
Meta-analysis, 2023
Pooled analysis
Eight randomized trials pooled.Total hair count favored the combination. Hair diameter did not reach significance. Methods and scales varied.
Network meta-analysis, 2023
Pooled analysis
27 trials, 1,110 adults.Hair density at 24 weeks. Minoxidil plus microneedling ranked highest. Microneedling alone ranked below minoxidil alone.Indirect comparisons.

How to read mixed results

  • Small pilots often report bigger effects than large trials. The 2013 pilot had 100 men. The 2026 trial had 245 women.
  • The pooled analyses came out in 2023 and 2024, so they do not include the 2026 trial.
  • Needle depth, schedule and the partner drug differ between studies, so no single protocol is established.
  • Sex differs too: the pilot enrolled men and the 2026 trial enrolled women. We cannot say whether that explains the gap.

What prudence says

Do not apply any drug to a freshly needled scalp without talking to the person who prescribed or recommended it. Skin that has just been punctured behaves differently from intact skin, and the FDA says it has not reviewed combining microneedling devices with other products.

Questions for your clinician

  • Is a drug part of my plan, and who is responsible for it?
  • If I microneedle, when is it acceptable to apply anything to the scalp afterward?
  • What result at 3 and 6 months would make us change the plan?
  • What is the evidence for this exact combination in someone like me?

Common questions

Why are microneedling and minoxidil studied together?

An expert review says microneedling is thought to help first-line medicines penetrate the skin, and that this may be one way it acts. That is a proposed mechanism, not a proven one.

Can I apply minoxidil right after microneedling?

Ask whoever prescribes or recommends it. The FDA says it has not reviewed the safety or effectiveness of using microneedling devices together with other products, and it lists stinging or itching when products are applied among the less common effects. We give no timing, doses or products.

Does adding microneedling to minoxidil make a difference?

Results conflict. A 2013 pilot in 100 men and two pooled analyses favored the combination. A 2026 trial in 245 women found no significant difference after 24 weeks.
